Entering a nursing home is a very difficult decision for both residents and their family members. To facilitate a smooth transition and to help with the many emotions associated with placement, two social workers are available to provide one-on-one adjustment counseling.
They also facilitate a monthly Family Support Group that enables family members to provide ongoing support to one another. Every other month, Social Services coordinates the Family Council, where family members can discuss the current status of the nursing home with the Executive Director.
They also guide family members on how to obtain financial assistance through the State of Connecticut. For help on how to proceed with applications for financial assistance, Social Services will work with family members and the Nathaniel Witherell Business Office.
At times, residents do return home or are transferred to a less structured setting. The social worker will assist with implementing a safe discharge plan.

The Social Service staff at The Nathaniel Witherell is effective in welcoming new residents. They foster trusting relationships and express an ongoing interest in the residents' well-being. For family members, two forums are available on a monthly basis. A Support Group meets to offer emotional support and fellowship. Every other month, a Family Council is held with the administration to provide updates on relevant issues.
In addition to counseling, the Social Service staff performs a number of services for residents and families:
Discharge Planning - Increasingly, applicants look to The Nathaniel Witherell for short-term rehabilitation purposes. Social Service staff coordinates services to ensure safe discharge to the home.
Room Transfers - Residents are encouraged to express their individual needs. Sometimes, this leads to an expressed desire for a room transfer. Social Service staff coordinates arrangements to promote social compatibility.
Shopping - Independence and self-esteem are promoted relative to accommodating residents desire to purchase personal items and appropriate attire, which at times is specialized. The Social Service staff is available to assist in catalog purchasing upon request.
Community Living - Residents can look to the Social Service staff to promote positive relationships with peers. Social workers identify personality strengths, which are incorporated into the individualized care plans.
