St. Luke's Community Services, Inc. dba Luke's LifeWorks (SLLW) is soliciting proposals from qualified and certified/licensed contractors to replace the roof on its three family residential facilities located at 28 Rose Park Avenue, Stamford, CT 06901. Proposal packets containing basic information and the specific scope of the project are available at the SLLW office, at 141 Franklin Street, Stamford, CT 06901. A $10 nonrefundable fee for a proposal packet is required. A mandatory property/site visit (walk through) is scheduled at 28 Rose Park at 1:00 P.M. on November 1, 2010 (Monday).

Work to be performed under this contract will be partially funded by federal money through the Community Development Block Grant of the Town of Greenwich and as such, compliance with applicable federal laws and regulations are required (i.e. Davis Bacon Act, Conflict of Interest, etc.). Pursuant to CGS § 12-430 (7) a nonresident contractor shall furnish the Department of Revenue Services a guarantee bond for 5% of the total amount of the contract price using form AU-766. All proposals are final after actual date of receipt.

Proposal Requirements: Interested individuals or companies must submit three copies of the proposal (one original, two copies). Submitted proposals must be dated and signed by the CEO (if a company) or the individual (if a sole proprietor). The proposal must state the following information: company or individual information (i.e. name, address, TIN, etc.); fee structure; references; and work history. In addition, a copy of the applicable State certification/license and a copy of the certificate of insurance must be attached.

Criteria for Evaluation: Proposals will be evaluated based on the following criteria: content and quality of the proposal submitted, proposed price, level of expertise, level of confidence in proposed safety management, work history and references.

Proposal Deadline: Completed proposal packets must be submitted to and received by SLLW no later than 2:00 P.M. on November 15, 2010 (Monday). Each proposal is to be accompanied by a completely filled out and properly executed copy of the contract documents, including all forms. Proposals that lack the required information or attachments will be deemed incomplete and will not be considered during the review/evaluation process.
